To fix “Lookup failed” error, there are two options as below.

Option 1 – Update Mods with Latest Strings

Visit – https://www.nexusmods.com/fallout4/mods/97/?tab=files

Download latest strings.

Go to Fallout 4 directory.

Click on “Data” folder.

Open the download file.

Open the Data folder in the archive.

Drag the files to your Fallout Data folder.

Option 2 – Uninstall Fallout 4 Mods.

  • Open Fallout 4.
  • Go to main menu.
  • Select Mods.
  • Go to Library and find the mod you wish to uninstall.
  • Select the Mod and click on “Delete” (You can also disable it from menu).

Note: Players can be experience different scenarios while playing game with mods. If you wish to download and operate the mods, you can do it at your own risk.

You should confirm the Integrity of game files after uninstalling your mods.

It can be possible that above mentioned steps cannot fix this issue, so you need to uninstall complete game and reinstall it.
